Please be advised that Yellow Pad Prescription with Booklet Number 08A-012393 registered to Dr. Melissa Paulita V. Mariano was reported lost since May 3, 2013. Any transaction therefore using this prescription should not be honored.

You may report to the Dangerous Drugs Board or Department of Health any attempts to use Yellow Pad Prescription with the abovementioned numbers.

On 30 July 2012, they will officially be enlisted as members of the BKD after a launching seminar where officers and resource speakers from the Dangerous Drugs Board will provide participants with basic concepts and information on drugs and its prevention.
